Biography
Dr. Jelena Lewis is an Associate Professor of Pharmacy Practice at Chapman University School of Pharmacy (CUSP) and the Director of Residency and Fellowship Programs at the school. She is also an Advanced Practice Pharmacist and a Board-Certified Ambulatory Care Pharmacist. In addition, Dr. Lewis is a faculty in residence at Providence Medical Foundation in Orange County, CA where she developed and implemented a chronic disease management program.
Dr. Lewis received her bachelor's degree in biochemistry, with a minor in mathematics, from the University of California Los Angeles. She received her Doctor of Pharmacy degree from the University of Southern California, where she was a member of Rho Chi Academic Honor Society in Pharmacy. Dr. Lewis completed her PGY-1 pharmacy practice residency with an emphasis in ambulatory care and community pharmacy practice at the University of Southern California. During her residency, Dr. Lewis was an Adjunct Assistant Professor of Clinical Pharmacy. Upon completion of her residency, she worked as a clinical pharmacist at Cedars-Sinai Medical Care Foundation. During her time at Cedars-Sinai, Dr. Lewis worked in the drug therapy management department where she managed patients with chronic conditions such as diabetes, asthma, dyslipidemia, hypertension, and provided travel and smoking cessation consultations. She helped expand the Cedars-Sinai Injection Center which focuses on cost savings for injectable medications and vaccines.
In addition to her clinical work, Dr. Lewis is a preceptor for pharmacy students as well as pharmacy residents on their clinical rotations. She is a past APhA-APPM Medical Home/ACO SIG coordinator and past chair of the APhA-APPM Medical Home/ACO SIG Education and Resources Committee. Currently, she remains active within the APhA community.
Her research interests include pharmacist run chronic disease management program implementation and outcomes in an ACO/PCMH and pharmacy student leadership development.
